Cat Adoption Application

Animal Rescue Family (ARF) seeks responsible pet owners for our animals. Owners need to fulfill emotional, physical, veterinary and safety needs of their pets. Once adopted, dogs and cats must not be free-roaming pets. Adopters must be at least 21 years old. ALL adults in the household must participate in this adoption application and process.

By signing this application the applicant(s) gives permission for ARF to investigate any/all information provided by the applicant(s). Applicant(s) also give permission to inquire and obtain veterinary medical records for current and past pets. Furthermore, ARF will contact the current landlord for any dwelling they may rent to confirm permission for application to house an indoor pet. ARF reserves the right to deny any request to adopt a pet. Furthermore any misrepresentations made in the application may result in the applicant/adopter losing the privilege of adopting or keeping an adopted pet.

In the event we are unable to approve your application for adoption, we will not be disclosing details of our decision.

All required (*) questions must be answered in order to have a complete application for us to approve. We will not review incomplete applications!

  • Animal Rescue Family (ARF) makes a substantial effort to assure that adoptable animals are in good health. It is possible, however, that undetectable illnesses or conditions may be present or incubating at the time of adoption. Therefore, ARF does not make any guarantee as to age, behavior, breed, health, temperament or ultimate size of any animal. This animal is being adopted in AS IS condition and without guarantee. Whatever preventative treatment given by ARF to protect the animal's health is not conclusive and further precautionary veterinary treatment may be necessary.
  • It is the adopter's responsibility to secure and pay for services of a veterinarian of their choice. ARF is not liable for any veterinarian bills that are incurred after you adopt the animal and have it treated by a veterinarian.
  • The Adopter understands that upon taking possession of the animal, he/she assumes all liability for the animal and any injury or damage, which may occur to or because of the animal.
  • Adoption fee includes spay/neuter with our veterinarian, vaccinations provided up to adoption and medical attention provided up to adoption.
  • The Adopter hereby indemnifies and saves harmless the ARF, board members, directors, employees and volunteers, from all losses, damages, costs, expenses, liability, claims, actions and judgments of any kind whatsoever, including without limitation, attorney's fees and litigation costs, arising out of or caused by any damage or injury inflicted by the animal once the adopter has taken possession.



Cat Addendeum



We may not be able to approve an adoption if we feel our animal's needs are not compatible with a prospective adopter's profile. All animals remain the property of Animal Rescue Family until an adoption contract has been properly completed, and the adoption fee has been paid. This screening form does not constitute an agreement to adopt. We reserve the right to decline an application for adoption for any reason. If we must decline an application for adoption we are under no obligation to disclose our reasoning.
